Technology, safety and improved quality of life for seniors and other vulnerable individuals.
Our Veterans Liaison can help determine a veteran’s resource needs, provide education and information regarding the various programs and services that Veterans are eligible for, and refer and connect veterans to the appropriate services.
Our team also includes Transitional Care RNs for assistance:
- Medication management/safety
- Assess if appropriate for homecare
- Recent hospitalization or ER visit
- Cognitive concerns
- Complex medical history
- Frequent falls
- Nutrition education
- Disease specific education
- Family dynamics/dysfunction
