You have the right to:
- Get a copy of your paper or electronic medical record
- Ask us to correct your paper or electronic medical record
- Request confidential communication
- Ask us to limit the information we share
- Get a list of those with whom we’ve shared your information
- Get a copy of this privacy notice
- Choose someone to act for you
- File a complaint if you believe your privacy rights have been violated

For certain health information, you can tell us your choices about what we share.
In these cases, you have both the right and choice to tell us how to:
- Tell family, close friends, or others involved in your care about your condition
- Share information in a disaster relief situation
- Include your information in a hospital directory
Except as permitted or required by law, in these cases we never share your information unless you give us written permission:
- Most marketing purposes
- Most sharing of psychotherapy notes, specialized substance abuse program records, and HIV-related testing and treatment
- We may contact you for fundraising efforts, but you can tell us not to contact you again

We may use and share your information as we:
- Treat you
- Run our organization
- Bill for your services
- Help with public health and safety issues
- Do research
- Comply with the law
- Respond to organ and tissue donation requests
- Work with a medical examiner or funeral director
- Address workers’ compensation, law enforcement, and other government requests
- Respond to lawsuits and legal actions

Health Information Exchange
Hartford HealthCare Member Organizations currently participate in health information exchanges (HIE’s), which ultimately help enhance the quality of your care.
The goal of the HIE’s is to help participating physicians and providers give better, more efficient care to their patients by the sharing of health information across secure systems. This means that wherever a patient goes, the patient’s health information may be available to all doctors who use the HIE’s, which helps to provide safer, more coordinated patient care.
You may opt out of the Health Information Exchange by doing one of the following:
- We have chosen to participate in the statewide health information exchange, Connie. As permitted by law, your health information will be shared with this exchange in order to provide faster access, better coordination of care and assist providers and public health officials in making more informed decisions. You may opt-out and disable access to your health information available through Connie by calling 1.866.987.5514 or completing and submitting an Opt-Out form to Connie by mail, fax, or through their website at www.conniect.org. Public health reporting and Controlled Dangerous Substances information, as part of the Connecticut Prescription Drug Monitoring Program (PDMP), will still be available to providers.
- We also participate in CTHealthLink, another secure, electronic HIE. If you do not want your health information shared and used through CTHealthLink, please go to the following website CTHealthLink - Patient Opt-Out and complete and submit the opt-out form.
- Care Everywhere is a part of Hartford HealthCare’s Electronic Health Record (EHR). It has exchange capability with organizations and providers who participate. As permitted by law, your health information will be shared with other providers and organizations across the country. You may opt-out and disable access to your health information available through Care Everywhere by calling the Hartford HealthCare Office of Compliance and Integrity at 860.972.1573.
